Emergency Contact & Medical Information Form
All members should AT LEAST carry details of their next of kin and medical conditions/prescribed drugs in their first aid kit. Some Club members may wish to carry more comprehensive information about their medical conditions. This is entirely optional. For those members who wish to do this, click on the above link for a suitable form. This information can be handed to paramedics if members are involved in a medical emergency on a walk.
